I would probably not match into general surgery in the current environment as I had a single middle author paper when I applied. But I could evaluate patients, cut, tie, sew, and handle tissue reasonably well as a PGY1. Clinical excellence should be prioritized. Papers can wait.

It is strictly based on quantity, rather than quality, at most institutions. To some degree, same for academic promotion (hopefully quality factored more here though)
