Everyone in Canada deserves to have access to a team that includes their family physician. Best to have us all .. YOUR FP, NP, FP nurse, Pharmacist etc.. ALL on your team… but not fragmented care with multiple unconnected providers. The College of Family Physicians of Canada OCFP SGFP
